SIU Open Access Policy: Authorization Form

This form acknowledges acceptance of the Open Access Policy by a faculty member.  It is a one-time form only to be used to grant an individual the authority to submit articles to OpenSIUC on behalf of the faculty member.  Faculty submitting their own article(s) directly to OpenSIUC do not need to complete the form. 

The completed form should be sent by email to opensiuc@lib.siu.edu.
